Complete the following before your visit:
Create your Headway account
Add a payment card on file
Sign Headway’s digital consent forms
Headway will charge your copays or coinsurance automatically If insurance verification fails: We simply default to cash-pay
I Do Not Have Insurance

Intake: $150
Follow-ups: $120
Sliding scale possible (in cases of verified need)
No insurance card or Headway account needed
Once your forms are completed, your appointment will be approved
